Hemorragia intracerebral posterior a drenado de hematomas subdurales crónicos
Resumen
Introducción: el hematoma intracerebral posoperatorio tras el drenado de hematomas subdurales crónicos es una complicación muy rara vez reportada, sin embargo, su incidencia, según diferentes series podría estar subestimada.
Caso clínico: se describe un paciente masculino de 77 años quien tras el drenado de hematomas subdurales crónicos bilaterales desarrolló hemorragia extensa en los tálamos, ganglios basales, mesencéfalo y protuberancia, con extensión hacia las cavidades ventriculares e hidrocefalia obstructiva.
Conclusiones: la compresión ejercida por la colección extraaxial disminuye el flujo sanguíneo cerebral sobre los hemisferios afectados y altera su autorregulación vascular. El rápido aumento del flujo sanguíneo cerebral en áreas cerebrales con alteraciones de la autorregulación vascular parece ser el mecanismo precipitante de las hemorragias intracerebrales tras la evacuación quirúrgica de los hematomas subdurales crónicos.
