Impacto de la función renal retardada en la sobrevida del injerto renal de donador de cadáver

Resumen


Objetivo: determinar si la función renal retardada (FRR) se asocia con disminución en la sobrevida del injerto renal y cuáles factores predicen su desarrollo. 

Métodos: se estudiaron 58 receptores de injerto renal de donador de cadáver. Se consideró FRR del injerto al no descenso de la creatinina plasmática en la primera semana postrasplante, requerir más de una sesión de diálisis o volumen urinario menor a 1 L/día por más de dos jornadas consecutivas. 

Resultados: la curva de Kaplan-Meier y Log rank demostraron que FRR > 15 días se asoció con reducción en la sobrevida del injerto (Log rank = 4.15, p = 0.042). El análisis de regresión logística demostró que el tiempo de isquemia fría > 12 horas (OR ajustada = 7.99, IC 95 % = 2.36-27.04, p = 0.001) y la diálisis > 3 años (OR ajustada 1.67, IC 95 % = 1.14-3.47, p = 0.032) se asociaron con FRR del injerto. 

Conclusiones: FRR > 15 días se asoció con reducción en la sobrevida del injerto renal y favoreció la presencia de otros factores que reducen la sobrevida del injerto. El tiempo de isquemia fría > 12 horas y el tiempo en diálisis de los receptores > 3 años favorecieron FRR del injerto.
