In the last decades, medicine has experienced intense technological, political and cultural changes that have reconfigured the role of the physician. To characterize the actual status of the medical profession, this article discusses the crisis in the medical profession through the identification and analysis of 6 macrosocial phenomena that can be traced through the sociological perspective including commercialism, managerialism and medicalization. Aside of these tendencies, it seems that it is in the daily encounter of the physician and the new role of the patient where these changes are reproduced and strengthened. The analysis nourishes from a sociological perspective by eliciting the classical attributes of the profession: monopoly of knowledge, auto-regulation, mandate and license. In the light of this concept, it becomes clear that the first three attributes suffer an involution, which offers insight to reexamine the grounds in which the concept was constructed, and to reflect on the implications for medical education.
En las últimas décadas, la medicina ha experimentado grandes cambios tecnológicos, políticos y culturales que han reconfigurado el rol del médico. Con el objetivo de caracterizar el estatus de la profesión médica actual, el presente escrito discute la crisis que enfrenta la profesión a través de la identificación y el análisis de seis fenómenos macrosociales que pueden rastrearse desde la perspectiva sociológica, incluyendo el comercialismo, el gerencialismo y la medicalización. A la par de estas tendencias, se arguye que es en el encuentro cotidiano del médico con el nuevo rol del paciente donde se reproducen y reiteran estos cambios. El análisis se nutre de una perspectiva sociológica al contrastarse con los poderes o atributos clásicos de la profesión: monopolio del conocimiento, autorregulación, mandato y licencia. A la luz de este concepto, se vuelve patente el declive de los primeros tres, lo que da pie a reexaminar el concepto de profesión a la luz de la teoría social y reflexionar sobre las implicaciones que trastocan la educación médica.
