Descripción en cadáver de nueva incisión segura para osteosíntesis de fractura de acetábulo

Resumen


Introducción: las fracturas de acetábulo son lesiones complejas por su conformación, localización y estructuras que lo rodean, por lo que requieren grandes incisiones para su osteosíntesis, con riesgo elevado de lesión de estructuras importantes. Se propone una nueva incisión para acceder al acetábulo realizada en cadáveres, para determinar si se trata de un corredor de seguridad anatómica factible para osteosíntesis.

Métodos: en cadáveres adultos se practicó incisión vertical de 7 cm en la unión del tercio medio y el medial, de la espina anterosuperior del iliaco hasta el tubérculo del pubis. Se disecó hasta el peritoneo y se identificaron las estructuras necesarias para la osteosíntesis de acetábulo.

Resultados: en siete cadáveres adultos, seis masculinos, se realizó la incisión bilateral para identificar todas las estructuras necesarias para la osteosíntesis. No se encontraron estructuras anatómicas importantes en el trayecto de la incisión. Se pudo colocar placas suprapectíneas e infrapectíneas con fijación inversa.

Conclusiones: existe una nueva incisión a través de un corredor de seguridad anatómica por la cual es posible realizar la osteosíntesis de acetábulo, con menor riesgo que otras incisiones.
