Mesotelioma pleural. Similitudes clínico-radiológicas entre estirpes
Resumen
Objetivo: describir las características clínicoradiológicas del mesotelioma pleural de acuerdo a las estirpes histológicas.
Material y métodos: se revisaron los expedientes de pacientes hospitalizados en el Instituto Nacional de Enfermedades Respiratorias durante los últimos 11 años con el diagnóstico de mesotelioma pleural.
Resultados: de 85 casos el tipo histológico más frecuente fue el epitelial (84.7 %), seguido del sarcomatoso (12.9 %) y del mixto (2.3 %). Al comparar las estirpes epitelial y sarcomatosa, no observamos diferencia en edad (53.7 ± 13.1 vs. 55.9 ± 11.0 años), relación hombre:mujer (2.3:1 vs. 1.8:1), antecedente de exposición a asbesto (34.7 vs. 27.2 %), tabaquismo (54.2 vs. 45.4 %), ocupación, tiempo de evolución (4.8 ± 3.3 vs. 4.4 ± 3.7 meses), dolor, disnea y tos, frecuencia de predominio derecho (55.6 vs. 81.8 %), imagen radiológica con derrame (59.7 vs. 36.4 %) o engrosamiento pleural (38.9 vs. 63.6 %) y eficiencia diagnóstica de la biopsia pleural cerrada (58.3 vs. 27.2 %).
Conclusiones: nuestros resultados sugieren que las características clínicas y radiológicas de las estirpes epitelial y sarcomatosa son muy similares. Un hallazgo adicional fue la alta frecuencia del mesotelioma epitelial, que contrasta con lo publicado en otros países y sugiere que el tipo de asbesto o de otros factores involucrados en el desarrollo del mesotelioma pleural son diferentes a los que existen en otras regiones del mundo.
