Resumen
Introducción: la enfermedad por SARS-CoV-2 presenta un amplio espectro de complicaciones y repercusiones cardiovasculares, renales y cognitivas. Los principales hallazgos cardiovasculares son pericarditis, miocarditis y trastornos del ritmo, descritos en un 5 a 11% de la población general. Sin embargo, en México no existe información concreta sobre las repercusiones cardiovasculares de la infección en personal sanitario.
Objetivo: identificar los hallazgos ecocardiográficos post-infección por SARS-CoV-2 en médicos residentes de un hospital de concentración de tercer nivel.
Métodos: Se realizó un estudio descriptivo. Se seleccionaron 62 médicos residentes con diagnóstico de COVID-19 en un periodo de marzo 2020 a febrero 2023. Se les realizaron ecocardiogramas transtorácicos y posteriormente se describieron los hallazgos obtenidos.
Resultados: se reclutaron 62 residentes, 38 hombres y 24 mujeres. Los principales hallazgos ecocardiográficos fueron: refringencia del pericardio (90.3%), derrame pericárdico (6.5%), movilidad del ventrículo izquierdo anormal (1.6%), función diastólica anormal (8.1%), insuficiencia aórtica (6.5%), insuficiencia mitral (19.4%), insuficiencia pulmonar (35.5%), tricúspidea (80.6%), FEVI media de 65% y PSAP de 23 mmHg.
Conclusiones: los hallazgos ecocardiográficos más relevantes fueron pericardio refringente, derrame pericárdico, disfunción diastólica del ventrículo izquierdo e insuficiencia valvular, predominio en hombres.
Abstract
Background: SARS-CoV-2 disease is associated with a wide spectrum of cardiovascular, renal, and cognitive complications and effects. The most common cardiovascular findings are pericarditis, myocarditis and arrhythmias described in 5-11% of the general population. Specific information on cardiovascular outcomes in healthcare workers in Mexico is lacking.
Objective: To determine the echocardiographic findings after SARS-CoV-2 infection in residents at a tertiary care teaching hospital.
Material and methods: A prospective, cross-sectional, and selective study was performed. A sample of 62 resident physicians with a diagnosis of COVID-19 was selected from March 2020 to February 2023. Transthoracic echocardiograms were performed, and the findings were described.
Results: The main echocardiographic findings were: pericardial refractoriness (90.3%), pericardial effusion (6.5%), abnormal left ventricular mobility (1. 6%), abnormal diastolic function (8.1%), aortic regurgitation (6.5%), mitral regurgitation (19.4%), pulmonary regurgitation (35.5%), tricuspid regurgitation (80.6%), mean LVEF of 65% and PSAP of 23 mmHg.
Conclusions: The most relevant echocardiographic findings were refractory pericardium, pericardial effusion, left ventricular diastolic dysfunction, and valvular insufficiency, predominantly in men.
