Fascitis plantar crónica: efectividad de los estiramientos, ultrasonido y sonoforesis. Un ensayo clínico aleatorizado

Resumen
Introducción: la fascitis plantar (FP) es la causa principal de dolor en el talón. El manejo conservador es amplio, pero no hay un consenso sobre la terapia física adyuvante más eficaz.
Objetivo: comparar la efectividad clínica de la sonoforesis más estiramientos, el ultrasonido terapéutico más estiramientos y los estiramientos aislados en la reducción del dolor en pacientes con FP crónica.
Material y métodos: ensayo clínico aleatorizado, ciego simple, realizado en una unidad de rehabilitación. Se incluyeron 60 pacientes con FP crónica, asignados aleatoriamente a 3 grupos (n = 20 en cada grupo): G1: sonoforesis + estiramiento; G2: ultrasonido + estiramiento, y G3: solo estiramiento. Se evaluó el dolor mediante la Escala visual analógica (EVA) al inicio y al final de 15 sesiones. Se utilizó la prueba t de Student pareada para la comparación intragrupo y ANOVA con post-hoc de Tukey para la comparación intergrupal.
Resultados: los 60 participantes (76.7% mujeres) completaron el estudio. Todos los grupos mostraron una reducción estadísticamente significativa del dolor (p < 0.01). La reducción media (EVA) fue: G1 (4.9 puntos), G2 (4.3 puntos) y G3 (6.7 puntos). El G3 (solo estiramiento) alcanzó el valor EVA final más bajo (0.9 ± 0.5). La comparación intergrupal mostró una diferencia estadísticamente significativa (p < 0.05) a favor del grupo G3 sobre G1 y G2.
Conclusión: los estiramientos aislados demostraron una eficacia superior en la reducción del dolor comparados con la adición de ultrasonido o sonoforesis en el tratamiento de la FP crónica.
