Determinantes clínicos de dosis de levotiroxina ajustada por diversos pesos corporales

Resumen

Introducción: el hipotiroidismo requiere tratamiento con levotiroxina para alcanzar el estado eutiroideo. La dosificación basada solamente en peso corporal presenta limitaciones, especialmente en pacientes con sobrepeso u obesidad.


Objetivo: evaluar si la dosis de levotiroxina requerida para alcanzar el estado eutiroideo, ajustada por diferentes pesos corporales, varía según sexo, edad, índice de masa corporal (IMC) y tipo de hipotiroidismo.


Material y métodos: estudio transversal analítico en adultos con hipotiroidismo eutiroideos en tratamiento con levotiroxina, atendidos entre agosto de 2024 y agosto de 2025. Se excluyeron pacientes con ajustes recientes de dosis o condiciones que afectaran la absorción del fármaco. Se analizaron dosis absolutas y ajustadas por peso actual, peso ideal, peso ajustado y masa libre de grasa.


Resultados: se incluyeron 101 pacientes, 76.2% de mujeres y 23.8% de hombres, con edad media de 50.8 ± 14.9 años. Las mujeres recibían dosis mayores de levotiroxina ajustada por peso ideal y por masa libre de grasa que los hombres. Los pacientes con sobrepeso y obesidad recibían dosis mayores de levotiroxina total y ajustada por peso ideal que los pacientes con IMC normal. No hubo diferencias en la dosis de levotiroxina ajustada por cualquier peso corporal, por edad ni tipo de hipotiroidismo.


Conclusiones: el sexo influye en la dosis de levotiroxina ajustada por peso ideal y masa libre de grasa, mientras que el grado de IMC influye en la dosis total y en la ajustada por peso ideal.
