Manejo farmacológico de dolor en heridas crónicas y estomas en el paciente adulto

Resumen
El dolor asociado a heridas crónicas y a estomas es un problema clínico frecuente y complejo que es constantemente infravalorado e infratratado. Su fisiopatología es multifactorial, ya que combina mecanismos nociceptivos, inflamatorios y neuropáticos, lo que justifica la necesidad de un abordaje terapéutico integral y farmacológico multimodal. La presente revisión analiza de manera crítica la evidencia reciente sobre los tratamientos sistémicos, tópicos y adyuvantes utilizados para el control del dolor basal, el dolor secundario a procedimientos, el dolor neuropático y el dolor irruptivo en pacientes con heridas crónicas o estomas. Se incluyeron revisiones sistemáticas, ensayos clínicos y guías de práctica clínica publicadas en los últimos 5 años. Los analgésicos no opioides y los opioides continúan siendo la base del tratamiento farmacológico; sin embargo, su eficacia puede ser limitada cuando hay componentes neuropáticos no identificados o mal caracterizados. Las formulaciones tópicas de anestésicos locales, opioides y agentes emergentes han mostrado beneficios especialmente en el dolor asociado a curaciones y procedimientos locales. Asimismo, los gabapentinoides y los antidepresivos de acción dual resultan eficaces en el manejo del dolor neuropático, mientras que los opioides de acción rápida pueden considerarse para el dolor irruptivo en contextos clínicos seleccionados. Aunque la evidencia disponible es heterogénea, respalda un enfoque individualizado que integre distintos mecanismos de acción y considere las características de la herida o del estoma, las comorbilidades del paciente y los aspectos de seguridad clínica.
