Micosis fungoide en una serie pediátrica mexicana: características clínicas, inmunofenotípicas y terapéuticas

Abstract

Resumen


Introducción: la micosis fungoide (MF) es un linfoma cutáneo primario que en pacientes pediátricos con frecuencia se confunde con dermatosis benignas. Los estudios en niños mexicanos son limitados.


Objetivo:  describir las características clínicas, inmunofenotípicas y terapéuticas de la MF en una serie pediátrica mexicana.


Material y métodos: se llevó a cabo un estudio transversal en un hospital de la Ciudad de México entre 2017 y 2025, el cual incluyó 17 casos pediátricos menores de 18 años de ambos sexos con diagnóstico de MF.


Resultados: fueron 13 hombres y 4 mujeres, con edad media al diagnóstico de 11.9 ± 2.8 años y mediana de evolución de 3 años; el 52.9% fue referido con el diagnóstico de dermatitis atópica. La variedad más frecuente fue hipopigmentada (64.7%). Todos los casos se clasificaron en estadio IB. El diagnóstico requirió una mediana de 2 biopsias, con retraso en la toma de hasta 12 meses (rango 3-24). Todos los casos mostraron epidermotropismo con positividad para CD3 y CD4 (100%), y el 88.2% positividad para CD8. Todos los pacientes recibieron esteroides tópicos; 3 casos recibieron fototerapia, 3 metotrexato y 3 fotoféresis. La forma clásica de la MF se asoció con la presencia de manchas eritematosas y placas eritematoescamosas, así como, en algunos casos, con el uso de metotrexato y fototerapia.


Conclusiones: la MF pediátrica constituye un desafío diagnóstico por su similitud con dermatosis comunes; aunque el pronóstico es generalmente favorable, su alta recurrencia requiere un seguimiento prolongado.
