Características clínico-patológicas de casos receptor 2 del factor de crecimiento epidérmico humano 1+ en cáncer de mama

Resumen

Introducción: la expresión del oncogén HER2 tiene un objetivo pronóstico y terapéutico específico en la atención del cáncer de mama. Una expresión baja del gen llamada HER2-low implica la posibilidad de ser candidato a terapia dirigida. Sin embargo, existe una variabilidad importante en su interpretación y de ahí la importancia de enfatizar su adecuada identificación y reporte.


Objetivo: estimar la proporción de casos confirmados como HER2 1+ entre biopsias previamente reportadas como HER2 1+ y describir sus características clínico-patológicas, así como la concordancia interobservador en su clasificación.


Material y métodos: estudio observacional, retrospectivo y analítico de casos previamente diagnosticados como HER2 negativo score 1+, que fueron observados por 2 patólogos independientes.


Resultados: se incluyeron 351 casos con una edad media de 59.56 años. Los subtipos histológicos fueron carcinoma ductal infiltrante NOS (70.66 %), grado histológico 2 (56.92%) y SBR con puntuación de 6 (41.82%), asociados en un 89.17% a la expresión de receptores hormonales. Hubo concordancia para el diagnóstico HER2-low en 78.91% de los casos, otorgada por un kappa de Cohen simple de 0.62, el cual es similar a algunos estudios documentados en la literatura.


Conclusiones: la concordancia en la interpretación de HER2-low aumenta si se realiza por al menos 2 patólogos; asimismo, el adecuado manejo de las piezas quirúrgicas en la fase preanalítica es vital para una óptima valoración, ya que esto tiene una implicación directa en el pronóstico y el tratamiento en la actualidad.
